We offer a PDF document that can be printed for your clients that wish to complete the questionnaire manually (rather than completing it inside the online system). View the attachment below to access this PDF.
NOTE: Several different question formats are used in this discovery process (some questions can be left blank, while others have required answers). Since there is not a consistent format for answering all of the questions, this discovery process is not recommended for clients to complete this questionnaire using the manual form, we recommend using the DNA Platform.
After reading our recommendation above (in the section listed with NOTE) if you wish to continue to use this manual form,follow the below steps once you have confirmed that your client has completed the questionnaire, 100% correctly:
- Create the client using the Create New Client screen (or if the client already exists use the Click to Schedule button locate on the client detail page). Click here for instructions.
- When scheduling, choose the email option: "Send emails to" and choose to send the emails to an administrator's email address, rather than the client. Also, check the box: "Also send completion email to this email address".
- Once you have scheduled the client for the discovery process, you will receive the invitation email (rather than your client) in your administrator's inbox containing the log in details.
- Follow the steps provided in the email you received to log in as the client and then transpose the responses the client provided into the system for them.
